When will I receive my last paycheck?
Final pay checks are issued per final pay guidelines of the state that you work in.
For California based employees, this means that you’ll receive your final check on your last day
of work. A signature is required for FedEx to deliver payroll checks. If any final payments are
being sent to your home a home address, please be sure someone is there to sign for the deliver
to avoid any delays. For voluntary resignations without notice (i.e. resigning on the day of), your
final check will be available within 72 from the date the company is notified.
How will I receive my last paycheck?
Most final pay checks are issued as a live check, unless your worksite location state allows a
direct deposit. In some cases, this means we must obtain a written authorization via a form from
you before the check can be direct deposited. If this authorization is not obtained, or not
obtained on time to meet deadlines, a live check will be issued.
My company offers us PTO or accrued vacation time. Will I get paid out for any unused time? What about my sick leave?
If your company has a paid time off (PTO) policy or accrued vacation plan in place, and you have
any unused vacation time or PTO time, those hours will be paid out on your final pay check. If
company does not offer a PTO or vacation plan or offers time off that does not accrue (i.e. Open
PTO, Flexible PTO, Rest & Relaxation Time, etc.), you will not receive any type of PTO payout.
Any remaining sick time will be forfeited.
Will I have access to the Worklife Portal and Sequoia's Advocate Team after my last day of work?
Yes, your access to the WorkLife Portal for pay stubs, W-2s, etc. will remain in place even after
your last day. You’re also able to contact the Advocate Team at (415) 937-9299 or
garten@help.sequoia.com for any questions related to payroll, benefits, the WorkLife Portal,
etc.

What happens to my benefits?
Company sponsored group health benefits, such as medical/dental/vision, will end on the last
day of the month following an employee's termination date. Other benefits, Short-Term
Disability (STD), Long-Term Disability, Life Insurance, and Flexible Spending Accounts (FSA), will
all terminate coverage on the actual last date worked.
Employees will be able to extend coverage for the group health benefits (medical/dental/vision/Healthcare FSA) for up to 18 months after that by enrolling in COBRA.
Sequoia’s COBRA provider, Navia, will send information on enrolling in this coverage to the
home address listed in the WorkLife Portal within 14 days of your benefits ending.
How long will I have access to perks like OneMedical and Smart Spend after I leave?
OneMedical membership will end on the last day of the month in which an employee's
termination date occurs.
If you have a service agreement with a vendor such as SoFi, it will continue between the vendor
and yourself.
Access to all other Sequoia One perks ends on your termination date.
How do I elect COBRA coverage?
COBRA enrollment information will be sent to the home address on file in the WorkLife portal
with instructions on how to elect COBRA coverage. You’ll need to complete and submit the
election form to Navia no later than the Election Period End date listed on the COBRA Election
Form.

How long do we have to use the money in our FSA accounts and our commuter benefits?
Your Navia card will be deactivated on the date of your termination.
- Healthcare and Dependent Care FSA: You have until June 30, 2021 to submit claims for
eligible FSA expenses incurred through your last day of work. Any balances remaining
after that are forfeited. - Commuter: You’ll have access to use any funds available in your commuter account
through your last day. Any balances remaining after that are forfeited. Navia does not
accept claims for out-of-pocket commuter or parking expenses.
My company offers a 401k plan through Sequoia's Mass Mutual plan. How do I rollover those funds to a new plan/different account?
The rollover process works differently at each provider. For the most accurate information on how
to roll over your Mass Mutual 401k funds, please contact Mass Mutual directly at (800) 743-5274
or https://retire.massmutual.com. They'll provide the forms and instructions needed for you to do
so.
Please note there is an auto-closure clause if the balance in your account is under $500.
Will I get a W-2 from Sequoia One? If yes, when can I expect to receive it and how?
Yes, Sequoia One will issue a W-2 for the timeframe covered with Sequoia One. If your contact
information has changed, please ensure it is updated through the Sequoia One Worklife Portal.
If you have elected a paperless W-2, this is where you may access the form. If you have elected a
paper copy, it will sent to your home address in the system.
If I need a Verification of Employment after I leave the company, where can I find this?
Please contact the Advocate Team or HE at he@garten.co for any verification of
employment requests.
