Report a Food Related Incident

If you are experiencing severe signs and symptoms due to something you consumed, please immediately seek medical attention.

To report a food related incident, such as foodborne illness or found foreign object in our food product, please 'Submit a request' via the link above.

Foodborne Illness 

In the request, select 'Customer/Consumer/Client' and you will be prompted with additional fields to fill out. Anything with an asterisk are required information.

• Subject - Provide a brief subject line
• What brings you here today? - Select 'Safety' and then 'I would like to report a foodborne illness'
• Where are you located? - Select the location closest to you
• Please select the garten brand - Which of our services did you receive the product from
• Date & Time - What date and approximately what time did you consume the product
• Description - Describe what you ate (product brand, product name, etc.), amount eaten, when you first began noticing symptoms; the more details you can provide the better
• Attachement - Attach any photos that you may have of the package or menu

Please seek medical attention if you are experiencing any of the follow symptoms:

• frequent vomiting
• inability to keep liquids down
• bloody vomit or stools
• diarrhea for more than three days
• severe abdominal cramping
• signs of dehydration

Found Foreign Object

• What brings you here today? - Select 'Safety' and then 'I would like to report a foreign object found in my food'
• Where are you located? - Select the location closest to you
• Please select the garten brand - Which of our services did you receive the product from
• Date & Time - What date and approximately what time did you consume the product
• Description - Describe what you ate (product brand, product name, etc.), the object found; the more details you can provide the better
• Attachment - Attach any photos that you may have of the foreign object

If you have any allergy or dietary concerns regarding one of the products that you consumed from us, you can also submit a request to us via the same form.

• Description - Describe to us what your questions or concerns are regarding the product

Please seek medical attention if you are having an allergic reaction with any of the following symptoms:

• itchy eyes or face
• swelling of the mouth, throat, and tongue that's causing difficulty breathing and swallowing
• hives
• abdominal pain
• cramps
• repetitive vomiting
• severe diarrhea
• confusion or dizziness
